How Does Electrotherapy Work?

Definition and Basic Principles

Electrotherapy uses low-intensity electrical currents applied through electrodes placed on the surface of the skin. These electrical stimuli travel through the tissues to produce targeted therapeutic effects.

There are two main modes of action for pain reduction. High-frequency stimulation acts on nerve fibres according to the gate control theory, helping block pain signals before they reach the brain. Low-frequency stimulation, below 10 Hz, can trigger the production of endorphins, the body’s natural pain-relieving chemicals, providing longer-lasting relief.

Electrotherapy can also be used to promote muscle activation following surgery, for example. This treatment, known as Neuromuscular Electrical Stimulation (NMES), is designed to stimulate and reactivate muscle fibers after a traumatic injury or surgical procedure.

Effects on the Body

When an electrotherapy device is used, the electrical current passes through the skin and reaches deeper tissues. This allows it to produce three main effects: an analgesic effect that reduces pain perception, a muscular effect that stimulates contractions to strengthen or retrain muscles, and a thermal effect that improves local circulation. This non-invasive approach can be useful for both chronic pain, such as low back pain, osteoarthritis and fibromyalgia, and acute pain, such as sprains or post-surgical pain. Pain relief can often begin within 15 minutes of application and may last for several hours after the session.

Functional Rehabilitation and Muscle Strengthening

Beyond pain management, electrotherapy plays an important role in recovery after an injury or surgery. It can help maintain or restore muscle strength by producing contractions that prevent muscle atrophy, even when voluntary movement is limited. This technique can support the rehabilitation process by allowing muscles to be activated earlier during recovery.

Electrotherapy can also improve local blood circulation and tissue oxygenation, supporting tissue repair and optimal functional recovery.

When Is Electrotherapy Used in Physiotherapy?

Electrotherapy can be useful for decreasing pain in the back region, whether it is caused by muscle tension or structural issues. It may also be helpful for shoulder conditions, including tendinitis and post-surgical pain.

For knee problems, such as osteoarthritis or recovery after surgery, electrical current can help reduce pain while supporting muscle strengthening. These applications allow for progressive and well-tolerated functional rehabilitation.

Tendinitis Treatment and Other Applications

Tendinitis may respond well to electrotherapy, which can help reduce pain and support the tissue healing process.

What Are the Contraindications of Electrotherapy?

Electrotherapy has a few important contraindications that must be respected. It is not recommended during pregnancy, for people with a pacemaker, for individuals with epilepsy, for people with cancer, or in the presence of open wounds or metal implants in the treatment area. Electrical current may interfere with these conditions and cause complications.
